Tell us the address

A Westminster address can be in Adams County or Jefferson County. You do not need to know which one. The street address is enough, and we will tell you if we can come.

The AVA HomeCare office in Aurora is for scheduling and paperwork. Your caregiver comes to your house or apartment and lives and works nearby.

Help with the day

Some Westminster families need meals cooked at home. Some need laundry and light housekeeping. Some need a caregiver to stay and talk so the person is not alone for the whole visit.

Bathing, grooming, and dressing can be part of the same visit. These daily tasks are sometimes called ADLs. The person keeps doing the parts they can still do.

Caregivers also notice simple safety problems, like a rug that slides or a bathroom that is hard to see at night. They tell the family.

Ways to pay

Health First Colorado (Colorado Medicaid) may cover home care if you qualify. Your case manager decides which services are approved. We also work with families who pay privately or use long-term care insurance. Not sure which one is right for you? Call us, and we will walk you through your options.

A relative who cares for you may be able to get paid as your caregiver through AVA. Ask us how.

How care works

  1. 1. We meet with you at home.

Our care coordinator visits you and your family. We talk about your daily routine, what is hard now, and how you like things done.

  2. 2. We write your plan of care.

If you have Medicaid, your case manager decides which services you are approved for, and we follow that plan. If you pay privately or use long-term care insurance, we build the plan with you. Together with you and your family, we decide the order and the way each task is done.

  3. 3. Personal care and help at home, together.

We usually combine personal care, like bathing and dressing, with help around the house, like meals and laundry. This makes your day easier and helps you stay independent at home.

  4. 4. We train your caregiver.

Your care coordinator shows your caregiver how to follow your plan, step by step and in the right order.

  5. 5. Your plan stays the same until you want a change.

Your caregiver follows the same plan at every visit. If your needs change, just call us and we will update your plan together.
