Dementia care

Your caregiver comes to the Parker home

The AVA HomeCare office in Aurora is for scheduling and paperwork. Care does not happen there. We match you with a caregiver who lives and works near you.

Many families in Parker start with meals, light housekeeping, laundry, and companionship. A visit can include a cooked meal, clean clothes, and time together so the person is not alone.

We also help keep the home safe. Caregivers watch for things like a loose rug, a dark stair, or a chair that is not steady. They tell the family what they see.

Personal care includes bathing, grooming, and dressing. These daily tasks are sometimes called ADLs. The caregiver helps with the hard ones. The person can still do the ones they are able to do.

A free in-home assessment

Call or text 720.937.1500, or email info@avahomecare.com, and ask for a free in-home assessment. We visit, talk about the daily routine, and explain the next step. Phone hours are Monday through Friday, 9 am to 5 pm.

How care works

  1. 1. We meet with you at home.

Our care coordinator visits you and your family. We talk about your daily routine, what is hard now, and how you like things done.

  2. 2. We write your plan of care.

If you have Medicaid, your case manager decides which services you are approved for, and we follow that plan. If you pay privately or use long-term care insurance, we build the plan with you. Together with you and your family, we decide the order and the way each task is done.

  3. 3. Personal care and help at home, together.

We usually combine personal care, like bathing and dressing, with help around the house, like meals and laundry. This makes your day easier and helps you stay independent at home.

  4. 4. We train your caregiver.

Your care coordinator shows your caregiver how to follow your plan, step by step and in the right order.

  5. 5. Your plan stays the same until you want a change.

Your caregiver follows the same plan at every visit. If your needs change, just call us and we will update your plan together.

Ways to pay

Health First Colorado (Colorado Medicaid) may cover home care if you qualify. Your case manager decides which services are approved. A relative who cares for you may be able to get paid as your caregiver through AVA. Ask us how.
